Veterinary Diagnostics
We type submission forms in again at accessioning.
VetSci, in production since 2022 at University of Wisconsin Veterinary Care’s hospital laboratory, runs accessioning through reporting. Your first step is a thirty-minute conversation with an expert.
Submission forms are often typed in again at accessioning, field by field, before the case exists. FormSense reads each form, creates or links the records it names, and a person reviews every field before it reaches your LIMS. Start with two of your own submission forms.

The form is typed in again before the case exists
Specimens arrive from clinics, farms, and field veterinarians, each with a submission form written by whoever sent them. At the accessioning desk, what the submitter already wrote is typed in again before the case exists.
FormSense reads the form instead, and a person at the desk reviews every field before it reaches your LIMS. It is available now.
02Where it occurs
At the accessioning desk, where one form opens a case
One submission form has to resolve several linked records at once.
- The submitter and the owner.
- The patient or herd, and its species.
- The tests requested.
- The veterinarian, the party to bill, and the people who receive the report.
03Why it matters
The accession is where the case begins
Request items and tests are generated from the accession, results are captured against them, and the report goes back to the submitter the form named.
04Before changing it
List what each form has to resolve
List the submission forms you receive, and every record each one has to create or link. Decide which fields must be present before a case is accessioned, and who reviews a field that is missing or unclear. A missing collection date is held for review instead of guessed.
05Approaches
From a process change to a new system
- Change the process when fields are typed in that no step of the case uses. Enter only what accessioning has to resolve.
- Use a capability you already have when your laboratory system can already resolve the submitter, the owner, and the patient from its own lists.
- Connect the systems when the details already sit in a system your laboratory can reach. An interface carries them instead of a person.
- Add a focused tool when forms arrive on paper and will keep arriving on paper. FormSense reads the form and holds what it cannot read, and the person who typed the forms reviews the exceptions.
- Replace a system only when your laboratory system cannot hold the accession and the records it links.

08Getting started
Pick the size of work your intake needs
Start with the part of the workflow that needs attention. The work comes in three sizes.
- An assessment finds which records each form has to resolve and which fields must be held for review. That is our Understand phase, an on-site analysis at a fixed scope and price.
- A focused intervention changes one point of intake, with a process change, an interface, or FormSense on your submission forms.
- A broader change adds software, implementation, data migration, and the ongoing services you choose.
We give a price once the work is sized to your records, interfaces, and users.
